Problem:

Client cannot afford his medications.

Solutions:

• Screen for Extra Help Low Income Subsidy -Check to see if there’s a less expensive Part D plan or Medicare Advantage Plan on www.Medicare.gov -Check to see if there’s a less expensive pharmacy for their Part D or MA plan using www.Medicare.gov
• Check to see if there is a Patient Assistance Program to pay the meds on www.needymeds.org.   In STARS, check the box for Manufacturers Programs.
• Check to see if there is an organization that will pay for the meds on www.needymeds.org.   I’m STARS, check the box for Manufacturers Programs.
