There are several case-managed programs available to help older adults and individuals with disabilities receive the support they need at home, promoting independence, safety, and quality of life in their community.
The Community Care for the Elderly (CCE) Program helps functionally impaired older adults live in the least restrictive, most cost-effective setting. Services may include adult day care, housekeeping, home-delivered meals, home improvements, personal care, and specialized medical equipment and supplies.
CCE aims to prevent or delay unnecessary institutional placement. Eligibility requires being aged 60 or older and functionally impaired, based on an initial assessment and annual reassessments.
The Home Care for the Elderly (HCE) Program supports adults aged 60 or older and their caregivers in private, family-type homes.
Participants receive a basic subsidy averaging $160 per month for housing, food, clothing, medical or dental services, and other costs not covered by insurance. Eligible clients may also receive special subsidies for incontinence supplies, home health aides, and other in-home support.
There must be a caregiver who is 18 years of age or older and is physically present to provide supervision and assist in the arrangement of services for the client. The caregiver must live in the home with the client or have alternative arrangements for care to be assumed temporarily by another adult so that there is 24-hour supervision of the client.
The Alliance for Aging Family Caregiver Support Specialist coordinates services and connects families with resources as their needs change.
