Information & Referral Specialist

Position Overview

Immediate full-time opportunity for an Information & Referral Specialist in our Aging & Disability Resource Center/Elder Helpline.

The candidate will be responsible for the provision of information to individuals regarding public and private resources in the community, as well as referral of individuals to the resources best suited to meet their expressed needs. Follow-up shall be conducted on all referrals made.

Education, Skill Requirements and Essential Functions

  • Bachelor’s degree from an accredited college or university in a human service-related field, or an Associate of Arts degree from an accredited entity in a human service-related field
  • a minimum of two (2) years’ experience in information and referral services, case management, call center services, social services, or related work experience.
  • A high school diploma or GED and at least four (4) years’ experience in an interactive human services setting can be substituted for higher education degrees if applicable.
  • Ability to utilize crisis intervention style techniques to de-escalate verbal confrontations with difficult callers.
  • Ability to relate to older persons and be able to deal with the public effectively over the telephone and be a receptive, friendly listener. Knowledge of the local aging services network helpful.
  • Ability to work independently, as well as in a team environment.
  • Ability to handle telephone equipment while utilizing electronic software designed to access information.
  • Demonstrated PC ability to include Microsoft Office products, especially Word, Excel, Outlook, and experience using databases. Additionally, must be proficient using the Internet.
  • Ability to type a minimum of 35 words per minute on a PC and possess strong English grammatical and spelling skills.
  • Bi-lingual (English/Spanish or English/Creole) preferred.

Intake, Screening & Triage Specialist

Position Overview

Immediate full-time opportunity for an Intake, Screening & Triage Specialist the candidate will be responsible for assisting in the preliminary determination of programs and services best suited to serve the needs (long-term and immediate) of the individual caller. The primary responsibilities of the Specialist position involve intake, screening, and triage; however, all activities may be performed to meet the needs of the department.

Education, Skill Requirements and Essential Functions

  • Bachelor’s degree in a human service-related field from an accredited college or university, or an Associate of Arts degree from an accredited entity in a human service-related field.
  • Minimum of two(2)years’ experience as a caseworker, case manager, intake specialist, or related work experience with the long-term care client population. A high school diploma or GED and at least four (4) years’ experience in an interactive human services setting as a caseworker, case manager, intake specialist, or related work experience with the long-term care client population may be substituted for higher education degrees, if applicable.
  • Strong communication skills, attention to detail and organizational skills.

Family Caregiver Support Specialist

Position Overview

Immediate full-time opportunity for a Family Caregiver Support Specialist The candidate is responsible for providing caregiver support and care coordination as well as providing public education and outreach services to the local community. This position is also responsible to promote the efforts and increasing awareness of the Alliance for Aging/Aging & Disability Resource Center through training, educational and outreach events, presentations, and speaking engagements to the local community, professionals, and other organizations. Essential to this position are excellent oral and written communication, presentation, and training skills to support and promote the various components and responsibilities of the Elder Helpline/Aging & Disability Resource Center.

Education, Skill Requirements and Essential Functions

  • Bachelor’s degree in social work or counseling or a related human services field, preferably with a license in Social Work.
  • At least five (5) years’ experience in Human Services or a related field, preferably in Crisis Intervention. A combination of education and work experience will be considered.
  • Previous experience working with the public in an interactive human services setting is preferred with knowledge of the local aging services network helpful.
  • A background in customer relations and/or care management/services coordination is also preferred.
  • Must be able to handle confused, frustrated and/or callers with hearing and speech impairments and relate to older persons and the general public effectively over the telephone and in small to large groups.
  • Must be a receptive listener and be able to relate well to callers via the telephone and family members of all ages.
  • Thorough knowledge of community resources, eligibility criteria and application procedures for needed services.
  • Must be able to handle telephone equipment while utilizing electronic software designed to access information.
  • Must be able to develop and continue working relationships with public and private agencies.
  • Excellent problem solving, analytical, verbal, written and interpersonal skills.
  • Strong initiative and follow-through on responsibilities.
  • Exceptional attention to detail and organizational skills.
  • Ability to work independently as well as in a team environment.
  • Demonstrated PC ability to include Microsoft Office products, especially Word, Excel and Outlook. Additionally, must be Internet proficient.

Program Coordinator

Position Overview

Immediate full-time opportunity for a Program Coordinator in our Planning and Population Health unit, the candidate supports daily operations of various population health programs, including delivery of training and education, scheduling of programs and staff, client home visits, report preparation and document maintenance. Helps ensure contractual and programmatic requirements are met. Responsible for monitoring the activities of subcontractors and working closely with staff to develop special programs.

Education, Skill Requirements and Essential Functions

  • Bachelor’s degree in health education, social work, gerontology, nutrition, public health, or a related field.  At least two (2) years’ experience in Human Services or a related field. A combination of education and work experience will be considered.
  • Experience working with older adults is highly desirable.
  • Excellent problem solving, analytical, verbal, organizational and interpersonal skills.
  • Excellent communication skills, orally and in writing.
  • Strong initiative and follow-through on responsibilities. Exceptional attention to detail.
  • Ability to work independently as well as a member of a team, and exhibit flexibility and creativity in response to growth and change within the organization.
  • Bilingual, English/Spanish is a plus.
