Because the long-term services and supports (LTSS) system involves multiple funding streams and agencies, locating and accessing LTSS can be difficult and confusing. The Department of Elder Affairs (DOEA) is working with other agencies to determine how to make it easier for individuals to learn about and access the services and supports they need.

To assist in this effort, DOEA and partnering agencies are conducting a survey to obtain vital input from stakeholders. Stakeholders include service providers and advocates that assist individuals, families and caregivers to access services, and the individuals, families and caregivers, themselves.

The survey may be completed by service providers and advocates assisting individuals, families, and caregivers to obtain information about services and to access services. Additionally, the survey may be completed by “end users” (individuals, families, and caregivers).

The survey will be available through July 31, 2015. The information to be gathered on the survey is vital in helping DOEA to develop the NWD Three-Year Implementation plan.
